Information we collect

We collect information such as your name, contact details, scheduling preferences, and general messages you submit through forms or by email or phone. We also collect device and analytics data, including IP address and browser information.

How we use your information

We use your information to respond to inquiries, schedule appointments, operate our services, and comply with legal requirements.

How information is shared

We share information with service providers and telehealth vendors under Business Associate Agreements, and when otherwise required by law. We do not sell your personal information.

Your rights

You may request access to, correction of, or deletion of your information. You may also file a complaint with the U.S. Department of Health & Human Services, Office for Civil Rights.

Security

We take reasonable measures to protect your information. However, no method of transmission over the internet or method of electronic storage is 100% secure.

For a crisis or emergency, call 911 or the 988 Suicide & Crisis Lifeline. Please do not use website contact methods for urgent needs.
